On your first visit you will be asked to fill out the New Patient Packet so we can establish a medical history for you. You can expedite this process by downloading and completing these forms prior to your first visit. You may email or fax these forms to us. ALL PATIENTS MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN FOR THE FIRST VISIT- NO EXCEPTIONS.
